Search Instructions

1. There are ten fields on the search page, these ten fields are:
   1. First Name: The Doctor's first name
   2. Last Name: The Doctor's Last Name
   3. Practice Name: The name of the Practice
   4. City: The address of the Practice
   5. State: The State of the Practice
   6. Country: The Country of the Practice
   7. Area Code: The areacode for the Practice
   8. Financing: The financing offered by the Practice
   9. Procedures: The medical procedures performed at the Practice
2. Fill in any/all of the fields with the information you want to search for. Partial
   information is acceptable.
3. The system will then search for the information you
   requested and return the results.
